IS HOME HEALTH CARE RIGHT FOR ME?

Paying for your home health care

Home Health Care is covered under Medicare, Medicaid, Blue Cross/Blue Shield and many health care plans. Our team is available to verify your benefits, coordinate coverage and complete the billing process

TOP 3 REASONS TO REFER TO HOME CARE:
  1. Patient has a new diagnosis or exacerbation of chronic condition
  2. Patient has a fall risk (balance issues, weakness, new assistive device)
  3. Patient is at risk for readmission to the hospital in the weeks following discharge.
HOW SOON CARE IS STARTED BY CAREMUST:
  • CARE Must clinicians will Open patients within 24-48 hours of hospital discharge allowing for a smooth transition for a patient into their home setting
  • Assure that patients have a follow-up visit set with their physician within 5 days of hospital discharge or as advised by physicians or discharge plan.
  • Work with the patients and physicians on medication reconciliation
  • Assess patients for fall risk, depression, medication side effects/adverse reactions, and immunization statusCollaborate with other clinical partners and physicians and reinforce the care plan
HOW OFTEN I WILL RECEIVE VISIT FROM CARE MUST TEAM:

The frequency and type of home health visits and the services provided are based on your personal plan of care. Your doctor may change your plan of care, increasing or decreasing the number of visits or services provided, in order to provide you with the best home health care for your needs.

Home health is focused on helping patients recover from illness, injury, a medical procedure, or manage a chronic illness – all in the comfort and familiarity of their own home

WHAT IS THE DIFFERENCE BETWEEN HOMW HEALTH AND PERSONAL HOME CARE ASSISTANCE?
The difference between home health and personal home care assistance is the necessity for skilled services. Home health care requires skilled services that would come from a physician’s order. This includes things like registered nurses, physical therapy and occupational therapy.

Personal care assistance involves unskilled services that are typically paid for through private pay (not paid by insurance). These services include help around the house, transportation, bathing and some light meal preparation.
